Transformyou’s prescription-only Medical Emergency Kit provides you with a selected assortment of medications, preparing you for those unpredictable moments in life. If you are a new client/patient you will need to fill out a short intake form that our doctors will use to approve your suitability for the prescription kit. Existing clients/patients do not need to fill out this form.

Each kit is designed as an emergency supply for one adult. This is a prescription medication and you agree that the purchase of multiple kits is only intended for the person placing the order, not for any other individual(s).

Once the intake forms are completed where necessary and payment is made your order will be processed and you should receive your kit in 2-3 weeks. Transformyou is an Rx dispensing clinic and a majority of the medication will come directly from the clinic. Any compounded medications will be ordered from a compounding pharmacy.
